In 2014, Pathways of Washington was offered a contract with Washington State Department of Social and Health Services (DSHS) to provide Expanded Community Services (ECS). ECS started in Spokane County and has since grew to serving adults throughout the Eastern and Central Washington counties of Spokane, Whitman, Stevens, Asotin, Grant, Chelan, Douglas, Franklin, Walla Walla, Columbia and Benton.

 

What Pathways Can Do for You

Our services focus on behavioral support to assist the individual residing in assisted living facilities and adult family homes in maintaining their physical and mental well-being and overall quality of life. We do this through direct support and training to the individual as well as with the residential staff, providing them with the tools they need in order to keep individuals safe and to prevent decompensation that sometimes corresponds with behavioral health disorders. Many of our referrals also come directly from the State Hospitals. We also provide psychiatric assessments and psychiatric medication management by a Psychiatric ARNP. Crisis prevention plans and interventions are also woven into our service array.

As we continue to grow in our scope of service delivery, we have been offered an additional contract with DSHS HCS in proving Client Transition and Training. These services provide assistance to individuals in nursing facilities in transitioning to lower level of care, including independent living. We also will begin providing Challenging Behavior Services to individuals in nursing facilities who present behavioral challenges which limit their potential for otherwise residing in lower levels of care.

Our services are community-based, and we meet the client in their place of residence. As an organization, we are aware of the importance of exploring the individual’s perspective and strengths and the key roles of instilling hope, promoting self-help, spirituality, education, meaningful roles, the importance of medications and illness management, and the need to build supports for each individual as a “whole person” who is seeking a self-directed life. We utilize a team approach and working together cohesively, meeting gaps and needs, benefiting all involved. Our goal is to maintain quality community living for all served.
